[PROPOSED] ORDER TO ENTER JUDGMENT

This cause coming to be heard pursuant to the Agreed Motion for Entry of Judgment, and the Court being fully advised,

IT IS HEREBY ORDERED THAT:

1. The Parties' Agreed Motion for Entry of Judgment (Doc No. __) is granted; and

2. In accordance with this Court's Consent Decree entered December 21, 2011, the Clerk of the Court is directed to enter judgment against the Defendants, ordering and adjudging that Defendants pay a total of $1,200,000 for costs, expenses, and fees to Counsel for Plaintiffs in the following manner:

(i) Defendants shall pay to Access Living of Metropolitan Chicago a total of $976,568.02 in three equal payments of $325,522.67. Defendants shall make the first payment from State Fiscal Year 2012 appropriations (the fiscal year ending on June 30, 2012), the second payment from State Fiscal Year 2013 appropriations (the fiscal year starting on July 1, 2012 and ending on June 30, 2013), and the third payment from State Fiscal Year 2014 appropriations (the fiscal year starting on July 1, 2013 and ending on June 30, 2014).

(ii) Defendants shall pay to Equip for Equality a total of $85,926.99 in three equal payments of $28,642.33. Defendants shall make the first payment from State Fiscal Year 2012 appropriations, the second payment from State Fiscal Year 2013 appropriations, and the third payment from State Fiscal Year 2014 appropriations.

(iii) Defendants shall pay to the Roger Baldwin Foundation of ACLU, Inc. a total of $111,808.32 in three equal payments of $37,269.44. Defendants shall make the first payment from State Fiscal Year 2012 appropriations, the second payment from State Fiscal Year 2013 appropriations, and the third payment from State Fiscal Year 2014 appropriations.

(iv) Defendants shall pay to The Law Offices of Steve Gold a total of $25,696.68 in three equal payments of $8,565.56. Defendants shall make the first payment from State Fiscal Year 2012 appropriations, the second payment from State Fiscal Year 2013 appropriations, and the third payment from State Fiscal Year 2014 appropriations.

(v) Defendants shall complete and submit all paperwork necessary for the first payment, plus applicable statutory post-judgment interest, within (a) five business days after expiration of the time to appeal the Decree without the filing of a Notice of Appeal, or after the issuance of the mandate by the highest reviewing court, whichever is later, or (b) April 1, 2012, whichever is later. Defendants shall complete and submit all paperwork necessary for the second payment no later than July 1, 2012 and the paperwork necessary for the third payment no later than July 1, 2013.
